In order to help create the best experience for you, we have put together this important information in preparation for your first visit. Please read through each section and bring all necessary items with you to your appointment. See you soon!

Struggling with Morning Sickness?

Here is some information to help you deal with Morning Sickness.

Please bring the following with you to your appointment:
  • Picture ID
  • Health Insurance card
  • Payment for co-pay/co-insurance (all payments are due at time of service by cash/check/credit card)
  • If transferring from another physician, bring any medical records with you

At Which Hospital(s) Can I Deliver?

Each provider group has specific hospitals that they are credentialed to deliver at. It is the patient’s responsibility to check with their insurance to know which hospitals are covered by their insurance.

We encourage you to contact your insurance and ask them what hospitals you can go to with your insurance plan. Once you know what hospitals your insurance allows you to go to, you’ll want to make sure it is a hospital that your provider group delivers at. Our staff and providers are not able to tell you what hospitals you can go to with your insurance- we want to make sure that you have the correct information directly from your insurance to avoid any expensive out of network costs that can occur by going to a hospital not covered by your insurance plan.

The following is a general summary of the appointment schedule you can expect during your obstetrical care at our office.

How am I billed during my pregnancy?

The following is a general summary of what you can expect during your obstetrical care at our office and which services will be included in the physician fee (or commonly known as the “global fee”). The physician fee will be billed at the time of delivery. All services not included in the physician fee will be billed to your insurance at the date of service.

The billing department will contact your insurance after your first appointment to determine what your individual physician fee will be. A letter will be mailed to you before your second appointment to explain your benefits and estimate. You will be asked to make a payment on this amount at each visit during your pregnancy.
